3. Do not bring the questionnaire along during your observation as it may reveal your identity at the outlet.
4. Look out for the staff's grooming, personal hygiene, friendliness and courtesy rendered towards you.
5. Look out for the tidiness and cleanliness of the store.
6. In order to make the evaluation more effective ask as many questions as possible.
7. If time permits, inquire about other products as well and yet again, test the staff on product features, pricing, etc.
8. When possible, immediately complete the questionnaire after leaving the store when the shopping experience is still fresh in your mind.
9. All purchases made in this exercise will be borne by the shopper.
10. Please make sure all the questions are filled in and each box is ticked accordingly.
11. Your appointment and personal particulars are confidential. This will not be revealed to the staffs at the outlets, thus ensuring that they are at their most genuine performance at all times.
12. Late submission of feedback forms of more than one month will not be processed.
13. The company reserves the right to refuse issuance of vouchers of extra or late submission feedback forms.
